    This page will provide answers to many frequently asked questions, as well as links to commonly used medical forms.
     
    All 12th grade students are required to have a second MCV (Meningococcal Conjugate Vaccine). Students not compliant will not be permitted to start the school year.  This is mandated by the PA Department of Health for ALL students. This includes home school, cyber and in-person students
     
    All students K-12 must have an up-to-date health history each year. Health Form 2022 is available in the Sapphire parent portal. Please complete this form prior to 8/22. No standing order medications will be provided without this information.
     
    As always, if your student has a new health need that you would like to discuss with the nurse, please feel free to contact her. Reminder that all medications require a doctor's note and parent permission form. Students may not carry any medications with the exception of epi-pens and asthma rescue inhalers. If they plan to carry one of these, please provide a doctor's order.
     
    PA state requires each 11th grade student to have a physical. If your student has had a physical since July 2021, please have them bring it to the nurses office. Physicals may be e-mailed to Mrs. Miller at millerh@nebobcats.org
