Included in this folder are forms that may be presented to you during your child's school enrollment. Not all forms in this library are required. Our district strives for open communication between school and home. We encourage families to work closely with the health room and classroom to plan for their child's health needs. If you have questions or need assistance completing the enrollment paperwork, please contact the health room.

Allergy Checklist
Families with student(s) who have an allergy (food, medication, seasonal and/or environmental) will be asked to complete this form prior to the first day of school. The form should be submitted to the health room. The checklist is updated annually.

Asthma Checklist
Families with student(s) who have asthma will be asked to complete this form prior to the first day of school. The form should be submitted to the health room. The checklist is updated annually. It is also expected that if the student has an inhaler prescribed for as needed or regular home use, that the same medication (inhaler) to be provided to the health room for the school year. See NESD Medication Policy for additional details.

Diabetic Checklist
Families with student(s) who have diabetes will be asked to complete this form prior to the first day of school. The form should be submitted to the health room. The checklist is updated annually. Please see the NESD medication policies for additional information.

Emergency Health Concerns Form
All NESD families will be required to complete an Emergency Health Concerns Form for each student enrolled in the district. Completed forms and questions should be directed to your child's school health room.

Information about Potassium Iodide (KI)
Given the school's proximity to TMI, Pennsylvania offers students and school staff KI tablets to use in the event of a nuclear emergency. Parental consent is required for administration. Consent/declination will be obtained annually on the Emergency Health Concerns form.

Families with student(s) who have an allergy to insect stings will be asked to complete this form prior to the first day of school. The form should be submitted to the health room. The checklist is updated annually.

NESD Medication Policy
This policy accompanies the Medication During School Hours, medication order. The policy outlines responsibilities for medications to be safely administered during school hours.

Medication Order
Families with student(s) who have a chronic health condition that requires the use of medications during the school day, will be required to obtain a medication order from a physician. Medications may only be administered with this order and parental consent. Families are responsible for supplying the medication to the health room. Medication orders expire at the end of every school year. Orders for upcoming the upcoming school year must be dated after July 1st. This form is to be used for prescription and over the counter products.
